DVA Exercise Physiology: Treatment Cycle, Reporting and Items
A DVA treatment cycle starts with an initial consultation and ends with a report to the veteran's usual GP. The report can be submitted after eight sessions, and 12 sessions should still be provided before moving to a new cycle. The detail below is from DVA's own schedule of fees effective 1 July 2026.

Who the treatment cycle applies to
- Treatment cycle referral arrangements started on 1 October 2019.
- The cycle does not apply to exercise physiology for Totally and Permanently Incapacitated Gold Card holders.
- Treatment for a White Card holder must relate to an accepted condition, and eligibility must be established before treatment starts.
- The cycle does not apply to multi-disciplinary case conferencing (EP76 to EP78), the End of Cycle Report (EP90), the Request for Service (EP99), or services provided while the client is admitted to hospital.
How the cycle works
- Start with an initial consultation. Every treatment cycle must start with an initial consultation, and only one can be claimed per cycle. It includes completing or updating the patient care plan, must be delivered individually, and is expected to run at least 20 minutes. For Totally and Permanently Incapacitated Gold Card holders an initial consultation can only be claimed for a new episode of care or a new and unrelated condition with a new referral, and at most three in a 12 month period.
- Deliver the treatment. Subsequent consultations are delivered individually, are expected to run at least 20 minutes, and cannot be provided on the same day as any other consultation or session for the same client. A group session needs two or more participants under constant overall supervision with intermittent individual care, and also cannot be on the same day as another session for that client.
- Send the End of Cycle Report to the GP. The End of Cycle Report goes to the client's usual GP. It can be submitted after eight sessions of treatment, but 12 sessions should still be provided before moving to a new cycle. Where the client needs a shorter course and no further cycle is required, a minimum of two sessions must be provided before the report can be claimed.
- Claim the report. Only one End of Cycle Report can be claimed per treatment cycle, and only after the report has been submitted to the GP. It cannot be claimed for a client identified as Totally and Permanently Incapacitated on their DVA Gold Card.
The End of Cycle Report
DVA publishes an official End of Cycle Report template, hosted on its treatment cycle information page for GPs. epnote fills that official form in the browser, and also offers its own letterhead version.

Item codes
DVA's exercise physiology schedule of fees effective 1 July 2026 uses item codes in the EP series: EP10 to EP21 and EP26 to EP32 for consultations and sessions, EP70 and EP71 for telehealth and phone consultations, EP76 to EP78 for multi-disciplinary case conferencing, EP90 for the End of Cycle Report and EP99 for a Request for Service.
The fee amounts are not reproduced here. They change every 1 July, and a stale figure on a page like this would be worse than no figure. Read them from DVA's current schedule.
DVA forms an exercise physiologist can author
- There is no DVA template for a patient care plan. Clinical notes are enough if they contain the required content.
- A treatment cycle referral does not have to be on D0904. It can be on practice letterhead.
| Form | What it is for | Who authors it |
|---|---|---|
| D1328 | Treatment prior financial approval request, for fees above the schedule in clinically exceptional circumstances | Exercise physiologist |
| D0992 | Mobility and Functional Support products order form, for RAP items including a stationary exercise bike, pedal exerciser and small exercise equipment | Exercise physiologist |
| D9160 | Request for trial of equipment, exercise bike or knee scooter, three months only | Exercise physiologist |
| D0904 | DVA request or referral voucher | The referrer, not you. You receive it. |
| At Risk Client Assessment Form | Treatment cycle at risk assessment | The GP, naming you as the allied health professional |
Questions
- How many sessions are in a DVA treatment cycle?
- An End of Cycle Report can be submitted after eight sessions of treatment, but 12 sessions should still be provided before moving to a new treatment cycle. Where the client needs a shorter course and no further cycle is required, a minimum of two sessions must be provided before the report can be claimed.
- Do I need to send a report to the GP at the end of a DVA treatment cycle?
- Yes. The End of Cycle Report goes to the client's usual GP, and the EP90 item can only be claimed after it has been submitted. Only one can be claimed per cycle.
- Does the treatment cycle apply to every DVA client?
- No. It does not apply to exercise physiology services for Totally and Permanently Incapacitated Gold Card holders, and it does not apply to case conferencing, the End of Cycle Report, a Request for Service, or services provided while the client is in hospital.
- Can a White Card holder access exercise physiology?
- Yes, where the treatment relates to an accepted condition. Eligibility has to be established before treatment starts.
- What are the DVA exercise physiology fees?
- They are set in DVA's Exercise Physiologists Schedule of Fees, which takes effect on 1 July each year. This page does not reproduce the amounts, because a stale fee would be worse than none. Read them from DVA's current schedule.
- Does DVA need a new referral after each cycle?
- A new treatment cycle starts with a new referral and a new initial consultation. The referral can be on practice letterhead rather than form D0904.
