Exercise Physiologist Report to GP Template for Medicare Referrals

Under a Medicare chronic condition management referral you report back to the referring GP after the first service and after the last one. There is no official template for that report, which is why this page gives you the structure and a worked example instead.

When you must report back to the GP

MBS item 10953's explanatory notes require a written report to the referring practitioner after the first service and after the last service, and at other times when it is clinically warranted.

There is no Services Australia or Department of Health report template for the 10950 to 10970 items. The obligation is to send the report, not to use a particular form.

What the referral itself has to contain

The referral is the GP's to write, not yours, but it is worth knowing what makes it valid before you bill against it.

  • Written, signed and dated. An electronic signature is acceptable.
  • The referrer's name, and their practice address or provider number.
  • The reason for the referral and the relevant information about the condition.
  • It does not have to name a particular professional or a number of services.
  • It is valid for the period stated on it, or 18 months.

Template: the first service letter

  • Who the client is, and the date of the first service.
  • The referral: who referred, when, and for what condition.
  • What you assessed, with the measures named in full and their results.
  • What you found, in plain words.
  • The exercise prescription, described as activities.
  • The plan: how many sessions, how often, over what period.
  • What you want the GP to know or do.

Template: the last service letter

  • The period covered and how many services were used.
  • Baseline against final measurements, with the change.
  • What changed in daily life.
  • What the client will keep doing on their own.
  • Whether further sessions or a new referral are recommended, and why.

Asking for more sessions or a new referral

Medicare's allied health services under a chronic condition management plan are shared across allied health professions, so a request for more sessions is a request the GP weighs against the client's other allied health needs. Say what the remaining sessions would achieve and what measurement supports it.

Questions

Do allied health practitioners need to send a report to the GP under Medicare?
Yes. MBS item 10953's explanatory notes require a written report to the referring practitioner after the first service and after the last service, and at other times when it is clinically warranted.
When must the report be sent: after the first service, the last service, or both?
Both, and also whenever it is clinically warranted in between.
How many exercise physiology sessions does Medicare cover under a chronic condition management plan?
Allied health services under a chronic condition management plan are capped per client per calendar year and shared across all the allied health professions the client sees, so the number left for exercise physiology depends on what else they have used. Check the current figure with Services Australia rather than relying on a summary.
Can I email the GP report, or does it have to be faxed or sent by secure messaging?
The MBS notes require a written report and do not prescribe how it is delivered. Practices differ, so ask the referring practice what it accepts. epnote exports the report as a PDF or a Word file for whichever route you use.
What changed in the 2025 chronic condition management framework?
From 1 July 2025 the GP chronic condition management plan replaced the earlier arrangements, and the older allied health referral forms were retired without a replacement template. The referral requirements themselves are set out in item 10953's explanatory notes.

Everything on this page comes from one of these. epnote writes nothing clinical from memory.

What this page does not cover yet

  • The brief asks for the one-page structure to be validated with two or three GPs, and for a second worked example in knee osteoarthritis. Neither is in the repo, so the structure here is the one epnote's Medicare templates already use and there is one example rather than two.
