NDIS Report Checklist for Exercise Physiologists
Work down the list before you send. Each item is an element the NDIA's own plan reassessment guidance asks for, and the section named beside it is where that element belongs. Nothing here is epnote's opinion about what a good report looks like.

How to use this checklist
Tick each item as you confirm your report answers it. The list is held in your browser while you work and nothing you tick is sent anywhere.
The items come from epnote's own mapping of the NDIA's plan reassessment steps and its report writing guide onto report sections. epnote checks the same list automatically as you write.
0 of 18 ticked. Nothing you tick leaves your browser.

Language checks before you send
- Requests are in hours, not dollars.
- No gym or pool membership or access cost appears in a recommendation.
- Nothing reads as general fitness, weight loss or exercise.
- Every test name is written in full the first time.
- Units are in words: seconds, metres.
- The change in daily life comes before the number.
- The report makes no claim about meeting any funder's requirements.
Before you send
- A copy has gone to the participant.
- If it is going to the provider portal, the participant has consented.
- Dates read as 16 October 2026 in text.
- There are no empty cells where a measurement should be. Write not tested instead.
Questions
- What does the NDIS want to see in a therapy report?
- The elements its plan reassessment guidance names: what the goals were, the service delivered with its approach and frequency, functional capacity at the start with the measures used, progress against each goal, barriers, risks, other supports, and what you recommend next with its justification. The table on this page maps all of them.
- How do I show progress in an NDIS report?
- Give the baseline and the current measurement for the same test, and say what changed in daily life before you give the numbers. A measurement with no baseline cannot show progress, which is why the assessment report matters as much as the progress report.
- Should recommendations include the exact number of hours?
- ESSA's guidance is that a request should give the number of hours rather than the cost, so yes, state hours. epnote's NDIS templates never print a dollar figure.
- What happens if the NDIA does not accept my report?
- epnote cannot tell you how the NDIA will decide, and it makes no claim about outcomes. What this checklist can do is make sure every element the NDIA asks for has been answered before you send.
- Does a complete checklist mean the NDIA will fund the supports?
- No. It means your report answers the elements the NDIA's own guidance names. The decision is the NDIA's, and nothing on this page promises an outcome.
